If you have been treated for a urinary tract infection or prostatitis in the last 6 weeks DO NOT proceed with this request at this stage. Please undertake a new PSA test in 6 weeks time if this is the case.

I consent for my results to be shared with my preferred GP and I consent to Medical Imaging Partnership and Prime Health processing my personal data for the purpose of my care pathway

I am aware this is a prostate health check service and the results may reveal there is a risk of cancer being present, and recommend further treatment which may include prostate biopsies
